Social work is available at Senior Health Primary Care. The Social Worker works with the primary care providers to manage psychosocial issues related to aging and impacts wellness.
Social Work can be helpful in assisting patients and/or families:
- Manage chronic illness more effectively
- Increase supports to better manage in the home
- To better cope with grief related to change and loss as one ages
- With short and long term planning
- Financial counseling
- Link to community resources
- Housing
- Food/nutrition
- Fuel assistance
- Medication resources
- Transportation
- Emergency alert systems
- In home supports
- Companions
- Social stimulation/activities
- Adult day programs
- Respite
- To reduce risk
- With advance care directives (Durable Power of Attorney and Living Will)
- With facilitation of communication within family or support network
