Your First Visit

Your first visit to our office is extremely important. Plan on arriving thirty (30) minutes prior to you scheduled appointment time so that you can complete your new patient paperwork which will include a health and family history questionnaire that will be reviewed with you during your visit.

Please bring with you:

• A list of medications or your prescription bottles which you are currently taking (if any).
• Any prior evaluations or behavioral health history reports.
