Your first visit to our office is extremely important. Your knowledge of your health history can help us to care for you better. Please consider the following:
A preliminary treatment plan may be suggested, which may detail recommendations for further assessment, referral to a more appropriate setting, or ongoing treatment with Elliot Behavioral Health Services.
In order to best meet your needs, the highly qualified team of psychiatrists, psychologists, nurse practitioners, licensed/clinical mental health counselors and social workers will work together with you to evaluate your needs and develop a plan of care. So they may begin that process at your first visit, we ask that you please complete the attached form (s) in its entirety, to the best of your ability. Please bring the completed form with you to your first appointment. This is vitally important to your plan of care. Failure to complete the form(s) will likely result in the need reschedule your appointment. In addition, it would be very helpful if you would bring previous medical records. We have enclosed a form for you to sign allowing the release of your records to our clinical staff. Please bring or send that completed form to your primary care physician and any other clinician that may have information pertinent to your care. We hope that your experience here at Elliot Behavioral Health Services will exceed your expectations as we help you restore the balance you desire in your life.
